Standalone privacy counterpart. Competitor soft-404s /privacy into terms; Ember wires both.
We collect information you submit on intake and contact forms: name, email, phone, state of residence, age confirmation, recovery goals, medications, and health history.
Purpose: clinician review, care coordination, and support. We do not sell personal health information.
Sharing: reviewing clinicians, authorized operations staff, and partner pharmacies only when an Rx path exists.
