What this demo intends to collect, what it does not claim, and which subprocessors are still empty slots.

Who we are

Meridian Men’s Health is operated in-house by LastStop Village (Remi Soltys / JennAI). Legal entity: BACKFILL (MSO / PC not formed).

What we collect (when intake is used)

• Name and email you type into intake.
• State of residence.
• Age range and sex as asked on the screen.
• Health-screen answers (conditions, goals, height/weight if entered).
• Messages you send if a contact channel exists.

We do not collect payment-card numbers on this demo. Stripe is not connected.

What we do not do

• We do not sell intake, email, state, or health-screen data.
• We do not claim a live third-party advertising pixel on these merchant pages.
• We do not claim HIPAA-covered-entity status.
• We do not invent a privacy-officer mailbox. Customer service email: BACKFILL — publish a real inbox before checkout.

Sensitive handling without a HIPAA badge

Health information is sensitive whether or not a covered-entity analysis is finished. Until that analysis is done, we treat intake and health-screen answers as confidential operational data: not for sale, not for unrelated marketing lists, not for a public dump. That is a conduct statement, not a certification.

Cookies

This static demo may use essential cookies or local storage for session or form state. We do not claim live analytics IDs, retargeting pixels, or a live ad network. If those are added later, this policy must name them before checkout goes live.

Subprocessors

Subprocessors: BACKFILL. When Stripe, OpenLoop, and a pharmacy exist, they will be named here. They are not named now because they are not live. Do not read a product-class sentence as a named 503A pharmacy.

Retention

Retention schedule: BACKFILL pending entity formation and a real EMR. EMR is BACKFILL. Do not invent a records-retention years figure.
